The Home Association located in Tampa, FL, provides customized and high quality care to our residents in this 96-bed skilled nursing environment. We are seeking an experienced full-time Social Services/Medical Records Assistant to join our team of dedicated health care professionals. This compassionate and service-oriented individual ensures that the social and emotional needs of each resident are met according to facility programs, policies and practices. This position will assist the Social Services Director and the Medical Records Coordinator with various administrative duties.
This position assists our Social Services Directors by interviewing residents and families to obtain social histories, participating in discharge planning, providing information and assisting resident’s with Medicare, Medicaid and other financial assistance programs, assist with referrals for ancillary services and in grievance/concern resolutions and investigations, works with family members to ensure open communication, performs general office duties. Must be an advocate and promoter of Resident’s Rights, Dignity and Respect.
Education and Experience
High school Diploma required, some college preferred.
At least 1 year in a social or human services environment in a long-term care setting.
Knowledge of PASRR and MDS processes is preferred.
Must demonstrate knowledge of resident needs assessment and discharge planning.
Must have proficient computer skills and communication skills.
